Registered Nurse Diabetes Prevention: Health Assessment and Promotion (Part-time)

Mission:  To provide outreach and direct services to support identifying and controlling cardiometabolic syndrome, including diabetes, hyperlipidemia, and hypertension, among underserved or high-risk populations.

Pay rate: $52.00 (W2) or $55.00 (1099) per hour.

Work hours: 15 hours per week, flexible schedule Monday-Friday.

Location: Decatur, GA.

Duties:

  • Provide client management (blood pressure monitoring, control support, education, etc.) for persons diagnosed with primary hypertension. 
  • Diagnosis based on current Joint National Commission clinical guidelines for detection, management, and treatment of hypertension. 

This service may be collaboratively offered with private providers. Provide management, treatment, control, and monitoring of hyperlipidemia per hypertension protocol guidelines.

  1. Type 2 Diabetes: Client management (glucose monitoring, control support, education, etc.) for persons with the primary diagnosis of type 2 diabetes. Diagnosis based on current Joint National Commission clinical guidelines for detecting, managing, and treating type 2 diabetes. This service may be collaboratively offered with private providers. Provide management, treatment, control, and monitoring of hyperlipidemia per type 2 diabetes protocol guidelines.
  2. Diabetes Prevention: Delivery of the Diabetes Prevention Program (DPP) for persons at risk of diabetes. Districts that deliver DPP should demonstrate the pursuit of National Diabetes Prevention Program (NDPP) full recognition status. Activities are as follows:
  • Enroll a minimum of 15 participants per year (“Enroll” is defined as having a completed registration form and the individual attending at least 2 sessions).
  • Support community outreach events: recruitment of patients, conduct screening and provide health education and promotion activities.

Performance measures:

The following are objectives to be achieved:

  • Objective 1:  Increase the number of adults visiting local health department clinics who will receive blood pressure screening and education.
  • Objective 2: Increase the rate of adults enrolled in the hypertension management program (monitoring, treatment, and education) with controlled blood pressure.
  • Objective 3:  Establish baseline rates for adults with controlled cholesterol levels who visit local health department clinics.
  • Objective 4:  Assist in reducing the prevalence of uncontrolled hypertension through at least 2 quality improvement projects. 
  • Objective 5: Increase the number of adults visiting local health department clinics who will receive screening through A1C or blood glucose testing and education.
  • Objective 6:  Establish baseline rates of adults with controlled type 2 diabetes who visit local health department clinics and are enrolled in the diabetes management program (monitoring, treatment, and education).
  • Objective 7: Increase the number of sites offering interventions through CDC-recognized Diabetes Prevention Lifestyle Change Programs to individuals with prediabetes or at high risk for type 2 diabetes to XXX.
  • Objective 8:  Increase the number of programs and sites offering interventions for controlling type 2 diabetes.
