Job Description

Are you a dedicated and detail-oriented nursing professional with a passion for quality care? We're looking for an experienced MDS Coordinator (LPN/RN) to join our team and help ensure excellence in clinical assessments and compliance.

What You'll Do:

Conduct accurate and timely MDS assessments to support quality care and reimbursement.

Ensure compliance with all regulatory requirements and submission deadlines.

Oversee MDS data entry and transmission, resolving any validation issues.

Collaborate with interdisciplinary teams to improve patient care plans.

Provide reports and insights to the Director of Nursing (DON).

Support facility audits and surveys, ensuring regulatory adherence.

Stay up to date with Medicare and Medicaid regulations.

Assist with quality improvement initiatives related to MDS.

What We're Looking For:

Licensed RN, BSN, or LPN from an accredited nursing program.

Active and current RN or LPN license.

At least three years of clinical experience in long-term care.

Prior experience with MDS/RAI assessments.

Strong knowledge of Medicare and Medicaid regulations.

Excellent clinical assessment and organizational skills.

Ability to collaborate with various healthcare professionals.
