Shift: Night Shift | 6:30 PM – 7:00 AM
We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) to join a skilled nursing and rehabilitation facility on a 12-week contract. The ideal candidate will provide high-quality patient care under the supervision of a Registered Nurse (RN) and ensure adherence to facility policies and state regulations.
Key Responsibilities:Provide direct nursing care to residents in accordance with established care plans.
Administer prescribed medications and treatments accurately and safely.
Monitor, record, and report changes in resident condition to the supervising RN or physician.
Assist with admissions, discharges, and transfers as needed.
Maintain accurate and timely documentation in medical records.
Collaborate with multidisciplinary team members to ensure optimal resident outcomes.
Uphold infection control, safety, and confidentiality standards at all times.
Respond to emergencies promptly and effectively.
Active LPN license in the state of Tennessee.
Minimum 1 year of recent experience in a skilled nursing or long-term care setting.
Strong clinical and communication skills.
Ability to work independently and as part of a team.
Current BLS certification (required).
