Position Overview: We are currently seeking a dedicated and compassionate Per Diem Home Health Nurse (RN) to join our team. This role involves performing skilled nursing visits, including wound care, medication reconciliation, and comprehensive assessments in the clients' homes. • Conduct skilled nursing visits in clients' homes.
• Perform wound care, including assessment, cleaning, and dressing changes.
• Complete medication reconciliation and ensure clients understand their medication regimens.
• Conduct thorough assessments to monitor clients' health status and identify any changes or needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide education to clients and their families on disease management and prevention.
• Maintain accurate and timely documentation of all client interactions and care provided.
• Communicate effectively with clients, families, and other members of the healthcare team.

Qualifications:

• Current RN license in the state of Florida.
• Minimum of 1 year of nursing experience, preferably in home health or a related field.
• Strong clinical skills in wound care and medication management.
• Excellent assessment and critical thinking abilities.
• Compassionate, patient-centered approach to care.
• Strong organizational and communication skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and a valid driver's license.
