Job Description

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Patient Care Manager in the Home Health sector located in Ormond Beach. As a Patient Care Manager, you will play a pivotal role in ensuring the delivery of high-quality home health services, coordinating patient care, and managing a team of healthcare professionals.

Key Responsibilities:

• Oversee the development, implementation, and evaluation of individualized patient care plans.
• Coordinate and manage patient care activities, ensuring compliance with clinical policies and procedures.
• Supervise, guide, and support nursing staff and other healthcare team members.
• Conduct regular patient assessments and ensure timely documentation and communication of patient status.
• Collaborate with physicians, therapists, and other healthcare providers to optimize patient outcomes.
• Ensure compliance with state and federal regulations and accreditation standards.
• Provide education and support to patients and their families regarding care plans and health maintenance.

Qualifications:

• Current Registered Nurse (RN) license in the state of Florida.
• Bachelor’s degree in Nursing (BSN) preferred.
• Home Health leadership experience required.
• Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or community health.
• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Strong organizational, communication, and interpersonal skills.
• Proficiency in electronic health records (EHR) and computer applications.
