Position SummaryThe Home Health Registered Nurse (RN) plays a vital role in providing skilled nursing services to participants in their homes. As a core member of the interdisciplinary team (IDT), the Home Health RN ensures participants receive compassionate, efficient, and person-centered care that aligns with their preferences and goals.
Key ResponsibilitiesConduct in-home nursing assessments to evaluate participant needs and equipment requirements
Collaborate with the interdisciplinary team (IDT) to develop and implement personalized care plans
Deliver home health nursing interventions, including medication administration, treatments, and health monitoring
Educate participants, families, and caregivers on disease management, preventive care, and self-care techniques
Facilitate seamless transitions between care settings (home, hospital, SNF) in collaboration with the WelbeHealth physician and IDT
Monitor and coordinate participant visits and schedules in partnership with the Home Health Manager
Graduate of an accredited School of Nursing
Unencumbered RN license in the state of practice
Minimum of 1 year of nursing experience in a clinical setting, preferably working with elderly or frail populations
Experience in home health care or community-based care preferred
Reliable transportation and valid driver's license required
