Position Summary

The Home Health Registered Nurse (RN) plays a vital role in providing skilled nursing services to participants in their homes. As a core member of the interdisciplinary team (IDT), the Home Health RN ensures participants receive compassionate, efficient, and person-centered care that aligns with their preferences and goals.

Key Responsibilities

• Conduct in-home nursing assessments to evaluate participant needs and equipment requirements

• Collaborate with the interdisciplinary team (IDT) to develop and implement personalized care plans

• Deliver home health nursing interventions, including medication administration, treatments, and health monitoring

• Educate participants, families, and caregivers on disease management, preventive care, and self-care techniques

• Facilitate seamless transitions between care settings (home, hospital, SNF) in collaboration with the WelbeHealth physician and IDT

• Monitor and coordinate participant visits and schedules in partnership with the Home Health Manager

Qualifications

• Graduate of an accredited School of Nursing

• Unencumbered RN license in the state of practice

• Minimum of 1 year of nursing experience in a clinical setting, preferably working with elderly or frail populations

• Experience in home health care or community-based care preferred

• Reliable transportation and valid driver's license required
